Please complete the following forms before coming to your initial evaluation and bring with you to your appointment. You may also submit the forms below prior to your initial evaluation. Please allow 30 to 45 minutes to complete the Intake Questionnaires.
An initial evaluation may take 45 minutes to one hour or more, depending upon the needs of the client. A full written report will be provided within 7 – 10 business days, complete with therapeutic recommendations. Once it’s determined if therapy will be helpful to you and your family, we can schedule therapy sessions.
It is preferred that children complete the evaluation, on a one-to-one basis, unless the child is under 3 years of age or has separation anxiety. We have a waiting room available for parents and siblings as needed.
